Hi,
I have an iPhone 4. For some reason when people call me, they can't hear me on regular speaker, or speaker phone. The mute button is not on. It doesn't work when I call them or when they call me. But if I FaceTime them, it works perfectly. But when I Bluetooth the phone to my car and call people, it does not work. I downloaded a "free phone call app" which works exactly like a phone does.. And my phone works where they can hear me! This just makes no sense. I would appreciate ANY help or advice you can give! Thank you in advance

Is your device jailbroken?
 
this just seems like a fluke... Some setting is off or something.. Go to settings, general, reset. and click reset all settings and see if that fixes the problem. if not. i will do a restore..
