So let me get this straight. Is the only difference between the subsidized ($199/299) and early upgrade ($399/499) pricing the amount of money "down" at the time of payment? Is the extra $200 inevitably paid off from being rolled into the monthly payments?
 
no, the 200 is for previous iphone users or other users renewing a 2 year with an available upgrade. The 200/300 and 400/500 both pay same in monthly

I understand the conditions for the 199/299 pricing. What I'm wondering is if people that qualify for that will eventually pay the same price for the phone as others over time.

No, they will pay a cheaper price and will have to wait 2 years to get discounted pricing again.
