Syncing iTunes and iPhone playlists

My playlists on my iPhone have been a disorganized mess for some time. In my iTunes library, they're all neatly gathered into folders, but most of them are outside of folders on my iPhone.

How can I duplicate the playlist and folder structure I have in iTunes on my iPhone? And why doesn't this transfer over with a simple sync?

It will if you have iTunes set up correctly. You have to tell it to sync music, and then tell it what music to sync.
