System Freeze whenever switching external monitor setup

So, in the office I have a single external 4k (dell), while in my home office I have a dual external 4k (LG, regular, not apple) set-up. HS now freezes whenever I from from home to office or vice versa. I've even tried disconnecting monitors before putting mac to sleep, and waking mac before connecting monitors. I'm on a late 2013 mbp 15 with discrete gpu.

Sierra never complained... it just re-synced and carried on.

Anyone else got this issue?
