I had not heard of this before but a T-Mobile store rep just told my wife that she will have I pay 10$ to activate the SIM card that comes with the iPhone 6 if ordered from T-Mobile.

Can anyone confirm/dispute this (from experience ordering the iPhone 5s T-Mobile)

That's true if she ordered it from T-Mobile. Full price devices ordered direct from Apple avoid the $10 SIM card activation fee.

We ordered straight from Apple. And I want to activate a new account with tmobile... will she have to pay the $10 + what ever service she chooses?

There is no "activation" charge for Tmobile. What they do try to get you with is a BS "new SIM" fee. You can bypass it as the "fee", it is supposed to be fore new SIMS. Ask them to waive it. There is no reason to be charging $10 for a SIM card anyway.

No. She can use the SIM that comes with the phone. $10 is for getting a new SIM from T-Mobile.
