If your tethering option has disappeared then all you need to do is go to the Settings>General>Reset option and Reset Network Settings should cause your phone to reset and the option to reappear.

The greyed out ipcc file is an issue with the version of itunes you are running. You will need to run beta 5 for OSX to have the option for updating the carrier file.

Or get around it by...
Quick note: On a Mac just start Terminal (Applications > Utilities) and type

defaults write com.apple.iTunes carrier-testing -bool TRUE

ipcc support will be enabled again ;-)

I've tried everything listed here (resetting network settings, doing that stuff in terminal) nothing worked. I am running iTunes 9.2 and previously I had the new OS 4.0 on my phone but downgraded to 3.0.1 so I could tether again. I got all of this to work and got tethering on my phone using help.benm.at but once I plugged my phone into my computer the tether option disappeared. PLEASE HELP, I want to tether! Thanks
