If you have a credit card with a billing address in the US, you can open a US iTunes account using that card.
You used to be able to open an account using just a gift card (instructions here), but I don't know for sure if that's still possible. If it is, you could try to get someone in the US to purchase US gift certificates for you and send you the confirmation number.
