We have a system that sporadically brings up a "do you want to initialize this drive?" dialog upon login. Disk Utility thinks that the main partition is called "/" and also shows its mount point as "/" (see screen grab). Meanwhile on the desktop it's properly called "Bay 1".
("PPSAN1" is our fiber-channel SAN volume, which also mounts successfully.)
After a restart the drive would show up correctly as "Bay 1" and the OS doesn't bring any non-intialized drive dialogs. Happens both in 10.8.4 and after an update to 10.8.5.
I also looked through system.log and was unable to find any related errors (not that I know exactly what to look for...). Besides this error and odd naming in Disk Utility everything else seems to work normally, but it's obviously something that I'd love to resolve.
