Basically, the devicewear union that locks the smart cover in place...

I check kick starter everyday hoping that it will pop up...have I missed it, by chance?

Does anyone have any idea why we haven't seen a case like this yet?

I don't understand your question.. I think Union is the case you are looking for..
 
When you fold the smart cover back, it is wider than the iPad.

It doesn't matter if they put a strong magnet on the back, it's always going to come off when you pick it up.

You’re better off folding the smart cover back on itself so it only takes up half the width, and holding it with that, or finding another cover.

Not quite understanding this. Several back shells already accomplish it properly, and I've just taped a piece of ferrous metal to the back of a Macally shell before and that holds the cover back without falling off.
 
Perhaps your case is wide enough that it mitigates the problem.

When you fold the smart cover back on the iPad and hold it with both hands, because the cover is wider than the iPad, it pushes itself off the device.

Personally I recommend avoiding the smart cover and either going with an integrated case, or a sleeve. I find the smart cover makes the device too thick, and the hinge is a problem when you're left handed.

Ah, I see what you mean. I guess I've just gotten used to holding it from hinge side since the ferrous metal/magnet on the back ultimately lets me do so.

As to the thickness, I'd say that unless you're going for something as thin as the STM Grip (thinnest full case I've got), I find the Smart Cover + Shell to be my thinnest option.
