Ever since Apple released Mac OS X Mavericks and removed skeuomorphism in the OS, there are three little things that bother me since then, because Apple just refuses to "fix" and it just shows how Apple is not the perfectionist it once was.

The first is the Trash. Whenever you drag an external drive to the trash, the icon changes and it used to change the name to "Eject", but now, it stays as "Trash". Maybe confusing newcomers and making them not trust this method?

Second, is the Keychain Access icon. The app has a new icon ever since, but to this day, if you open the app, look at the Dock, and you'll see the OLD icon, instead of the new one.

Third, it applies to iOS, too. If they made everything plain, why are we still having 3D wheels when selecting dates or times (for example). Go to Alarm app and choose the alarm, and it is clearly a 3D wheel. Why?

There might be more, but these are the three that bother me the most. Do you have any other little details that Apple has not done consistently with the OS or the behavior as it should be?

Bonus: There are few instances, where the old buttons show up in Apple webpage. One is the trailers page apple.com/trailers. They update the website everyday, but they haven't updated the UI.
 
1. I guess they forgot when they rewrote the dock.
2. It was fixed ages ago, probably in 10.11 or 10.12.
3. Because it can :cool: . iOS 7 was plain and a rushed job. macOS never went full "flat".

This is interesting, although I have never noticed as I typically eject drives a different way.

I can see how someone might be confused or concerned about this.
 
Just one distinction:
When dragging an external drive to the trash, I suppose one could have a problem with the word "Trash", but it's simple enough to explain - and the trash icon DOES change to an eject symbol. Not sure if most users would even notice the word, as it would happen pretty quickly.

@Ritsuka was spot in saying "macOS never went full flat". Open the Notes app, take a screenshot, zoom in on that screenshot and you'll see that the background is still a paper texture - this is easier to see in Dark Mode.
