Toggle shortcut between windows no longer works

All of a sudden, or so it seems, the ability to switch between different windows within the same application (i.e. Safari) via keyboard shortcuts disappeared on my late 2013 MBP - Mavericks.

I have looked at settings and turned on/off a few times.

Is there a fix?

Do I need to upgrade to Yosemite?
