TomTom's UK app was finally updated with "iPhone 5 and iOS6 support" and one of the features is as follows:

WORKS WITH APPLE MAPS
When you find a place to visit using the Apple Maps app, TomTom can guide you there. Simply select TomTom as the routing app and enjoy world class car navigation (requires iOS6).

Can someone explain to me how to do that, because I can't see any way of "selecting TomTom as the routing app".

Haha. Murphy's law, found it 30 seconds after posting.

I too would like to know how this works as I have not been able to find this feature either!!

Tap the bendy arrow in the top left. Type in a start and end point (if required). Two the train icon at the top and then tap "route". You'll be given a choice between TomTom and any other routing apps you have installed.
