Toolbar issues- in all apps

I've been having a strange issue that I didn't really notice until now, but the thing is, when I try to add an icon to the toolbar in many applications ( Adium was the last one this happened it) it will put a blank box in, and it won't turn into an actual icon until I quit the application. I've actually been able to add more than one of the same icon, and when I restarted the app there were two icons. How does this happen? It's happened in Adium, iWork, and a few other applications I can't remember, but it's erratic, happening only occasionally.
