I just moved to a new Mac and I was wondering if it's possible to sync the playlists I had on my old Mac which are on my iPhone to my new Mac? If not, then can I transfer playlists from one Mac to another and how?

If you have an external hard drive, you could move the itunes folder to your hard drive and transfer over to your new one. That's what I did and it worked just fine.

I already transferred my music via home sharing but I can't do that for playlists.
 
when i switch computers i use the program copy trans, merges numbers of plays, playlists, everything thats on your phone.
