I updated my iPhone 11 Pro Max today to 13.1.3 and now am unable to add accessories to my Home. The devices connect to network with no issue, but when I go to add it in Home (or the manufacture's app), it fails. I have 4 devices I'm trying to add and after I restart my iPhone and reset the accessory, connect to wifi, then go to add, I actually see all four devices show up for a brief moment then disappear when adding manually.

When I enter the code manually, it takes a long time and then fails.
 
Is your iPhone 11 Pro Max connected to a 2.4 GHz Wi-Fi network when adding these accessories?
 
Yes. My phone is connected to my 2.4GHz SSID.
I was able to get it to add the few new accessories last night using the app Control. It was a hit or miss but finally got it.

I am getting a lot of “ no responses”, randomly on many of my accessories. I ordered and will be installing a Mesh network next week to see if it improves. However my Linksys AC1900 has been rock steady when I was on IOS 12.
 
I experienced the same issue yesterday. It was the first time that I added a HomeKit device (Wemo plug) since updating to iOS 13 on my iPhone 7. I made sure I was on my 2.4GHz wifi band. I was able to set up the plug in the Wemo app, but the Home app never found the device when I tried to add it using the HomeKit code. Eventually, I tried using my iPhone 5 (which is running a version of iOS 10), and it worked with no issues. The Home app detected the plug as a new device, and I was able to add it using its HomeKit code almost instantaneously. It makes me think there's a bug in the latest version of iOS 13.

(I actually didn't think to use my iPhone 5 until I read this original thread post and wondered if it was an iOS 13 bug as well, so thanks for posting.)
 
I wish had an older active phone. I was going to use my wife’s 10 XS Max, but just upgraded her iOS to 13.1.3 for her new 11Pro Max that arrived. She has so many photos on her phone that I deleted the backup
so I could backup her’s with the new IOS 13.
Let’s face it, Home has bugs. And the way some manufacturers implement their HomeKit products are problematic. Leviton is difficult to upgrade at times. The most robust for me has been Netatmo cameras and VocoLinc outlets. The Yale Assure locks work well, but the app does not support IOS 13 as of today. There would be no way to add their locks to Home if you needed to.

it is a heavy investment to go with HomeKit, it needs to be solid.
