Trouble erasing USB flash-drive

Hey all,
Didn't know where else to ask this question and figured out this is the best place for it.

I have a 4GB USB flash drive that I want to erase. Problem is, Disk utility fails in the erasing process, and even more frustrating is that I tried erasing it using Terminal and it failed again.

The error I got while trying to erase the disk is "Couldn't unmount disk".

Anyone knows how to solve this issue..?
