Trying to send in iPhone to Apple to get fixed. Only giving me bluetooth options?

I'm trying to send my 1st gen iPhone back to get repaired and on the description of the problem, it only lists "AC Adapter/Cable/Dock is not functioning properly" and two problems with the stereo headset mic, and speaker.

I'm not sure what to do, because neither of those choices are my problem (touchscreen issues). Does anyone have any suggestions on what to do?
