I have everything turned on and i have signed out, deleted the app but to no avail. Push Noticiations worked fine with the native twitter app but I bought the ipad app when it was on sale. This weekend I liked it so much, I bought it for my iphone as well.

Maybe i'm not setting them up right? How do you set push notifications by each person you follow?

You set notifications for each of your accounts within Tweetbot.

You stated you turned everything on but, double check here -> Open app, tap Settings, scroll to Account Settings, tap your Twitter name, then tap Notifications, you then should get asked by iOS to enable notifications. Then enable each notification option.
 
I've done all that. I guess I thought you could set push notifications per each person you follow? At least I don't see it. The twitter client had this.
 
Push to WHERE? the app or to your email or somewhere else?

A notification banner. Just like native twitter did.

Example. I want a push notification when macrumors has a tweet. Only MR though.

Sorry but I can't seem to find the setting in the iOS Twitter app that allows you to select which tweets from followers you will be notified by.

click on a person you follow to see their profile
click on the person icon that on my iphone 5 is in the middle left hand side
Turn on notications.

Tweetbot is fantastic and in my opinion, the only missing feature.
