UK printer rebate - Has anyone tried it?

I'm wondering if anyone has tried purchasing a printer with their mac under the UK printer rebate scheme.

The offer on the page linked above is vague and the rebate form is not available.

Can you please tell me whether this is worth the hassle. What documentation do they require the get the rebate?
