Uninstall Address Book Dates 1.3.2. Help needed...

Hey guys,

Does anyone know how to uninstall Address Book Dates 1.3.2?
I've just installed it, but cannot get it to work in my Address Book. I guess it is not working very well with Mac OS 10.5.6

How can I uninstall it? There is no possibility to do so...
