upgraded iconoclasm and phone app is gone?

I downloaded myassistant and it was good then upgraded iconoclasm 2 mins later and it disappeared. Deleted both and it still wont come back

i type phone into spotlight and it comes up twice one with top hit next to it and the other is normal

weird it came back after i killed it in the multitask bar
