Far northern Maine.
I got my first iPhone two weeks ago. Love it!

But after literally 3 days, the USB Power Adapter went out.

Not a huge deal. I bought a new one at the Verizon store 70 miles away.

5 days later, THAT one died.

In both cases, they were sitting on my desk plugged in, and for no reason seemed to simply stop working. Both times, I didn't noticed until the phone went dead while supposedly charging, dropping my internet and Roku connections.

I can use the cable directly with the Mini's USB port, no problem.

Apple is sending me a replacement, but the tech seemed baffled.

Anyone else here had this happen?
 
You clearly have a problem with the quality of power in the power outlet you are plugging it into (possibly over voltage). You should likely have an electrician check the voltage output of the outlet you are using.

At a minimum, get a quality UPS with voltage regulation to regulate the power being provided to your electronics.

I still have all 8 adapters that have come with my families Apple iOS products over the past 7 years and every single one still works as well as they did out of the box. So, I really doubt that you have had several faulty adapters directly from Apple. If the adapters are 3rd party adapters then that's another story.
