Using old SIM Card

I know this doesn't apply to the new iPhone 5, so please bear with me.

Theoretical question:

If I own an iPhone 4, and upgrade to the 4S after one year at semi-subsidized price. Does AT&T/Apple provide you with a new Sim card inside the phone, or are you expected to keep your old one and put it in the new phone?
