I am seeing vast numbers of errors like this in system.log:

Code:
Apr 27 06:56:26 myhost /System/Library/Frameworks/CoreServices.framework/Frameworks/Metadata.framework/Versions/A/Support/mdworker[4542]: MDS Error: unable to create user DBs in /var/folders/Pq/PqE9RAlLEtCD7gLTGuawE++++TI/-Caches-//mds

  1. Do I need to worry about it?
  2. What causes it?
  3. How do I fix it?

I finally figured out what was going on here.

Apparently /var/folders/Pq/PqE9RAlLEtCD7gLTGuawE++++TI/-Caches-//mds is the personal mds cache directory for one of the users on my machine, and somehow it was corrupted. I just deleted the directory entirely, and rebooted. On the next login the affected user's mds [Spotlight] cache was rebuilt, and the problem has not recurred.

Spidey!!!
