iPad Verizon $200 offer

Anyone able to apply the offer for $200 with Verizon? According to the email from Apple you are supposed to fill out a rebate form, but I never received a form and when I asked Verizon over the phone they weren’t sure.

I already activated my iPad with Verizon but can’t find a rebate form anywhere to fill out.
