I have a 5S, this has been happening for a while now, at least the last 6-8 months, periodically. I would take a quick vertical pic (whether from lock screen, or launching the camera app), and when I view it in Photos, the orientation is horizontal. So when I'm viewing it as I took it, I see it as if it were a horizontal pic viewed vertically (small rectangle pic in the middle with black boxes at top and bottom, and pic is sideways). So obviously, if I turn my phone horizontally, it orients the pic back to full screen, but I have to tilt my head sideways to view it correctly. It's easily fixed by me going to edit and rotating it back to vertical. But I shouldn't have to do that, if it was taking pics properly. I didn't spend full pop on the 5S when it came out, to have it work half arse. Note, this has also happened when I take a horizontal pic, it oriented as vertical when viewed in Photos.
Anyone have this happen to them? What's the reason? Is there a "simple" fix? I've done a couple of updates since this started happening (once with having to install the iOS from scratch, along with all my apps...PAIN IN THE ASS!), and it's still happening. So I don't think updating to the latest version is going to change things. Is this another one of those things Apple likes to do to frustrate it's customers, so that they end up upgrading to the latest and "greatest" iPhone? For the record, this was also happening on my 4S. Like the 5S, not all the time, seems to be a random thing. It happens for Photo and Square capture modes. Happened once for video. Never for Pano.
