Voiceover On verbal messages when Voiceover is Not On

Hi. My 2013 MBP running Mojave keeps verbaally notifying me several times a day that Voiceover is on even though I have never turned Voiceover on and it is disabled in the accessibility settings. It’s very frustrating. Has anyone seen or heard this before and is their a fix? Thanks in advance.
