Volume coming out left speaker only several times a day

Okay it seems like several times a day that without going to system preferences the volume wants to come out of only the left speaker of my ibook. am i doing some unknown combination of keys that is causing the slider to move over withotu accessing the menu? its really annoying and happened on both ibooks ive used.

I've had this happen several times and honestly haven't a clue what has caused it. Only seems to happen on my laptop not the desktop.

I have this happen every so often maybe once a month or so on my Mac Pro.

I use external speakers. I think its a bug with OS X.
