volume icon in menu bar keeps disappearing with optical out use

When I plug in a mini-optical cable into the audio-out port on my unibody MacBook Pro he volume icon in the menu bar disappears. That is fine as there is no volume control needed from the system with optical out.

The problem is that when I unplug it the volume icon doesn't reappear. I have to go back in to System Preferences to "show volume in menu bar". I can also do this while the optical cable is plugged in, it's just that the volume slider is greyed out.

Any ideas on how to stop this odd behaviour?
