VZW Trade In Promo Code

When I go to fill out the trade in form for my Iphone 4, it is asking me for a promo code. When I received my confirm for purchasing the Iphone 6 did not receive a promo code. How do I get this? Without the promo code I do not get the $200 amount.

I just did it and did not need a promo code. There was an option for one, but not needed to get the deal.
