Not all cases will. Mine has a large opening around the lightning port to allow the adapter to work.

I would argue that when there are two adapters, where one is specifically designed to work with any case, changing the design of a case to fit the other adapter is slightly redundant.

As far as I'm aware, the 0.2m cord adapter doesn't carry USB audio, which rules out a lot of accessories. In my case that's a Traktor controller.

Yes, it does. The small little box and the 0.2m one are functionally the same.

Yeah what the guy above said. Worked great in my old car that only had the 30 pin iPod integration. The part that plugs into your phone is much thicker than the non-adapter OEM Apple Lightning cable, FWIW. It didn't fit with a couple of cases I had.
