Watch Passcode Lockout without entering Password?

I randomly got the password lockout warning on my watch (ie too many passcode attempts you need to reset your watch), without entering my password even once.

Anyone know how or why this could happen? Only thing I could think is that the watch was loose on my hand and kept asking for password, but dont know how it could have been entered wrong randomly the 10 or so times required for a lockout...

I would also guess the watch is too loose. It doesn't take much to randomly tap the screen 4 times in the general area of the passcode buttons.
