WebAIM recently got their survey results put together for how people use screen readers. It shows some interesting information and shows some good practices that we as web designers/developers should be aware of. This page gives some of the quick conclusions from the survey. Then the detailed write-up is found at this page. I found it worth the read and is an easy read.

Feel free to discuss as well. I found some of it a little surprising like how most respondents didn't know how accessible web 2.0 sites were, mostly caused by them not knowing what that was or whether or not they were on such pages. Also, Flash is still frowned upon by these users, though not directly attributable to Flash itself.
