Video: MP4, 1100 kbps 2-pass VBR
Audio: AAC 128kbps VBR

I use MPEG Streamclip. The settings I use depend entirely upon the purpose of the transcode.

Visual Hub for transcoding even though the dev no longer support it and Handbrake for DVDs obviously and use default settings for iPhone/ATV
