What causes flakey PDF highlighting in Preview?

I like using the highlighter in Preview in PDFs rather than printing a document and marking it up manually. But in recent weeks digital highlighting in Preview has been flakey. In the same document, in some places it works great to highlight a phrase, in other places it grabs seemingly random chunks of the document, and in yet other places it won't highlight at all. I noticed a similar problem in Office 2016. All this started in Yosemite and is continuing in El Capitan. Thanks.
