On the right is Thunderbolt 3 (USB-C) connector but what is the smaller on the left please? It is part of a small endoscope system I bought, which stated it was for iOS hoping to connect it to my iPhone 12 mini.
Maybe need an adapter?

A small endoscope then there is an adapter taking the Micro USB up to a standard USB.

I guess this would do?

Then I need an App to rune the Endoscope output?

For the software, check in the user guide for that endscope.
If it is sold as compatible with iOS, then there should be an app that is provided, or recommended.
There may also be support information at a web site for that endoscope?
 
What connector is on the other end of that cable with the USB micro connector?
The endoscope, hard wired into it so I need an iPhone (Lightening) connector that I can plug the Micro USB into.

I have a feeling it's only for OS and not iOS but I'll try.

These instructions say the device shows up like a USB webcam and can be used with any app that supports these, such as QuickTime Player on macOS. Are there apps to use USB webcams on iOS?
