What happens when you change stores in Itunes?

There is this game I want that is free for today only, and it is free in a different country. What happens when I get the game? I don't want anything else to delete. The game is for my Iphone and it is not free in the US state. Help Please!!!

nothing in your iTunes or iOS device will ever be deleted unless you approve the deletion.

I get music, games and apps from Japan all the time, it does not do anything like delete stuff when you change itunes store.
