Carrier What info do I need for AT&T?

I'm going to pre-order my wife's phone through the Apple website. She is on my family's shared plan (I am on a work plan). It's been three years since her last upgrade and I don't remember what information is needed. What, if any, account information will I need to buy her phone tomorrow?

Phone number, last 4 digits of account holder SSN, zip code.

just make sure you know the account pin (passcode)
it can be anywhere from 4 to six digits i believe.
The system might prompt you to input: a 6 digit code they send to any number on the account.
I just tried to do a mock upgrade with a number i don't use on my account using the apple website but hit a splashpage that says ("be right back. we're making updates to the apple store")

Maybe off here, but, shouldn't you be able to just order SIM-free and put in the existing SIM?
