What information do they ask when you do a pre-order on an upgrade?

Tommorow I plan on ordering three iPhones - all three AT&T upgrades and plan to order from apple.com. One of the phones is for my daughter who has her own account. What information do I need to get from her to place the order? I am ordering it for her because it is a birthday present and I'm paying for it, plus she is a school teacher and would have no time to place the order herself. The other two phones are for me and my wife. Thanks for any help.

You need the phone number for each line you want to upgrade, the last 4 digits of the SSN of the account holder (probably you?), and the zip code where each line is billed to.

Thanks. I am the account holder for mine and my wife's phones but my daughter has her own account. But I have her SSN.
