What information is needed to order iPhone through Apple?

Hi, I've never done this before but want to order the iPhone 5s online through the apple store at midnight. Now I will be adding a line to my Verizon share everything plan, and getting the phone at the subsidized price, but I am not the primary account holder.

So I'm wondering what info I need to get from my dad (he's the primary account holder) before I order. Like what does apple ask when adding a line to a share account (do I need his credit card? SSN?)

Thanks. Does it need to be his card or can I use a different credit card that's never been associated with Verizon? (Mine)

I have ATT and I can use any card associated with my Apple ID. Even if I had a brand new card I could simple add it to my Apple ID payment options to use it.
