I restarted and reset the PRAM.

I mean, you have two video cards, the IGP (Integrated Graphics Processor) called 9400M, and the dedicated GPU called 9600M GT. You currently have selected the either the 9400M or 9600M, but does it also happen with the 9600M or 9400M?
You can use gfxCardStatus to change between GPUs without logging out and in again.
 
OK, I installed gfxCardStatus and switched gpu's but no changes.

Also, why does the MBP have 2 video cards?

The IGP is there to conserve energy and the GPU can be used for me GPU intensive tasks.
Apple has used two graphic cards in their MBP line since the introduction of the Unibody design in 10/2008.

why not just always use both?

Just started today.
Color is distorted. Has very thin vertical lines.

MacBook Pro (15-inch, Mid 2009)
10.5.6 Leopard.
The problem on the MacBook of horizontal lines, it happened when you used the scroll-bar it will better to use track-pad it will help you to avoid this problem. And may be possibility of problem in driver issue of 9400M you can get help from support.apple.com/kb/TA27218?viewlocale=en_US

Because it is not designed that way. The Intel HD and 330M combination in the current MBPs works without logging out and in again and switches on the fly, but using both at the same time would be a waste of money for displaying normal content.
You don't really need two GPUs to write essays or watch Friendface videos.
