What's wrong with my iPhone 5?

For the past few weeks, it's been dieing at 5-10%... but whats weird is, I plug it back in to charge and it starts up the iPhone again and has 7% on startup.

Yepp, and one time at 39%.

sometimes the battery indicator isnt always on point. sometimes if you reboot, it will show the proper level. i usually tend to reboot the phone when i unplug it from a full charge just to make sure its accurate.
