How long until refurbs start hitting the Apple store? Has past history dictated something like 90 days after initial release? My mother-in-law wants a new iPad with 4G LTE, but if there will be refurbs in say a month, I'll tell her to hold off if she can.

No one's really going to be able to tell you that. and anyone that can shouldn't unless they like risking their job. It basically depends on how many iPads actually come back to apple via repairs or simple open box returns. I imagine they have to hit a certain number in order to have enough supply to both sell and replace customers units. You'll simply have to be patient and keep an eye on the site.
Macnn.com had an article on 8/31/2011 saying the first iPad 2's had arrived on Apple's refurb store.
