We've had this issue for years and Apple seems incapable or unwilling to fix it. Safari, is left to run for even a few hours without killing and reloading, will get to the point that pressing "+" to open a new tab (which then displays all of your bookmarks/favorites) will show blank gray images for each site instead of the icon for that site like it normally would. Killing Safari and re-running it solves the issue. But why is it still an issue? It appears to be a memory leak of some sort which should have been caught during testing yet somehow has been here through many iPadOS updates, including major releases.
