Where has the Airplay button gone ?!?!

I've been using all the iOS 4.2 betas and GMs on my iPhone 4 and enjoying Airplay with my Apple TV. I updated to 4.2.1 yesterday and—it may not be related—but I don't seem to be getting the Airplay button in any apps any more.

Airplay is enabled on the ATV, Wi-Fi is on for both devices, and Airplay works fine from my iPad.

Any ideas? I'm sure it's something basic I'm overlooking.
