From looking at the new training video, they show a companion activity app on the iPhone itself. I don't see that anywhere in the app store. Is this something that only appears when a Watch gets paired?

Good question. An older video on Apple's site calls it the "Fitness app" at minute 2:45.

Apple is usually very consistent with their marketing, they must have been polishing the software all the way up until launch.
Maybe the app just appears after you sync the Watch.
The activity is technically already installed if you have ios 8.2 and up, it just wont appear until a watch is paired.
