When I am in Safari on my Mac (but I believe it also happens on my iPad and iPhone, when I want to auto fill an address, the top choice is my previous old address. This address is still in my contacts but it the last address entry and isn't called "Home". I have poked around Safari and the autofill settings but I don't see where or why it has this old address. Anyone have any ideas of where it is coming from or how to delete it? The "Customize" option doesn't seem to do the job. The problem is, if I'm not thinking I pick it, it fills out the form with the old address and then I have to spend time correcting each field.

Just a thought. You might try removing the old address from your contacts, so the new address is the only one available. Perhaps it will then "acquire" the correct address from your contact record. If it does, you could try re-adding the old address, if you need to keep it for some reason. Then try autofill again, and see if it continues to use the new address. Be sure to check any auto fill data stored in Safari, as well.

you can update the card in the contacts app where it’s pulling the address from or make another card your card:
  1. Open the Contacts app
  2. Select Card > Go to My Card
  3. Select a card
  4. Choose Card > Make This My Card
