Is the Otterbox Defender the only one?
It's ok, but rather bulky. (The iPhone 4 version looks less bulky, though).

I hate applying the adhesive screen protectors, and they always seem to wear off.

Ivyskin will be coming out with some cases with built-in screen protectors. You can check out the 3GS versions to get an idea of what they will look like.
 
Get a good screen protector (like Zagg and Power Support) and not a cheap one.

Are they as difficult to apply as the more cheaply made ones? I always have difficulty getting the bubbles and "slick" out.
 
The dry ones can be applied wet and allows you to adjust and clear out the bubbles.
