How does one determine if a "banding" problem in the screen is a problem with the LCD or with the video card? Can the determination be made without swapping out parts?

Try with an external display and if it occurs on there more than likely graphics. If it doesn't points more towards LCD, but don't rule out bad graphics.
 
Thanks but I forgot to say this is an iMac, no easy way to hook up an external display

Well, the real question is whether you have an external display or not. An iMac is as easy to connect to one as any other Apple computer ist — you just have to stick the cable in ;)
