Screen protector is separate that I use a brand called JETech there protectors are great and cheap on Amazon also... the one on my X has lasted a year and hasn't cracked even after a few drops..the camera area of the case has a raised lip so the lense is not covered.
Unless it's one of those OtterBox/LifeProof/battery cases no reason to spend that much..I laughed when some shop at the mall tried to sell me a spigen case for 40 dollars plus tax.
